When you apply by email or mail, you can attach the receipt from File and Pay.

Please note: At this stage, File and Pay cannot be used to file documentation online for PSPLA tribunal applications. Note: For File and Pay please provide for Case title: Full Name and Date of Birth, and for Case number: PSPLA application/Permit number. |

Forms for updating licences and certificates
Annual Return Form
You can now submit your Annual Return Online.
The fee for filing an annual return is determined by the number of certificate of approval holders you employ, or currently contract. The fee is $40.00 per employee (if any) up to a maximum fee of $800.00. If you do not employ any certificate of approval holders, there is no fee for your annual return.
When to use this form
Use this form to submit your Annual Return to the Licensing Authority if you hold a current company licence or Individual license.

- Complete and attach spreadsheet
- Answer every question on the form unless the instructions tell you otherwise
- Credit/ Debit card
