Forms for the Immigration Advisers Complaints & Disciplinary Tribunal.

Section 54 Appeal form [PDF, 74 KB] Use this form if you want to appeal against the Immigration Advisers Authority’s decision not to refer your complaint to the Tribunal.
Section 55 Appeal form [PDF, 74 KB] Use this form if you want to appeal against the Immigration Advisers Authority’s decision to cancel your licence.
Statement of Reply form [PDF, 77 KB] Use this form if you disagree with anything in a Statement of Complaint that the Immigration Advisers Authority has referred to the Tribunal and you are either:

  • The adviser who is the subject of the complaint, or
  • The complainant who initiated the complaint
Answer to a Statement of Reply form [PDF, 72 KB] Use this form if you want to respond to a Statement of Reply that has been served on you by the other party to the complaint. You are not required to file an Answer to a Statement of Reply and should only do so if you want to provide a specific answer to any of the information or arguments provided in the other party’s Statement of Reply.
Additional Evidence or Witness Application form [PDF, 67 KB] Use this form if:

  • You would like the Tribunal to request further information from any person, or
  • The Tribunal has decided to hold an oral hearing and you would like to present oral evidence, or have witness present oral evidence.
Change of Personal Details form [PDF, 64 KB] Use this form if:

  • Your contact information changes during the course of proceedings before the Tribunal;
  • You authorise a person to represent you during the course of proceedings before the Tribunal; or
  • Your representative’s contact information changes during the course of proceedings before the Tribunal.
Application for Adjournment form [PDF, 69 KB] Use this form if the Tribunal has decided to hold an oral hearing and you are unable to attend on the day it has been set down or you would otherwise like the Tribunal to consider postponing the hearing. Adjournments are granted at the Tribunal’s discretion.
Witness summons [PDF, 94 KB] Use this form is you need a witness to give evidence at a hearing
